Hi Paul:
the reference may be Edward Amadeo, Keynes's Principle of Effective
Demand. Edward Elgar, Aldershot, 1989.  That is his Ph.D. dissertation
supervised by Milgate and Lance Taylor. The book has a preface by Vicky
Chick.
