It's not the lousy water which kills, but what's wriggling around in the lousy
water. Not sure if this answers your question, but the WHO has some pretty good
documentation for 20 water related diseases. Eg 2.2m dead from Diarrhoea in
1998 (about 6000 per day). Add in typhoid, cholera, dysentery, malaria etc. and
the bodies begin to pile up.

Presumably these deaths get attributed to a disease if it kills you before the
malnutrition does - WHO splits off nutrition issues to elsewhere on their site.
